In 1990, long before digital medicine became standard practice, Albertario led Winthrop University Hospital to become the first sleep center in the world to earn AASM accreditation without a single page of paper.
In the late 1980s and early 1990s, polysomnography was entirely analog. A typical overnight study produced a continuous roll of paper — often sixty feet or more — fed through a pen-based polygraph running at 10 to 15 millimeters per second. Technologists and physicians scored these studies by hand, spreading paper across large tables to review the night's recording. Storage rooms filled quickly. Sharing data between institutions meant physically shipping or copying reams of tracings.
The clinical and logistical limitations were significant. Paper degraded, signal fidelity was fixed at the time of recording, and there was no practical way to apply computational analysis to a physical tracing. The field needed a different approach — and a handful of early adopters were beginning to see what digital recording might make possible.
As Chief Technologist at Winthrop University Hospital's Sleep Disorders Center beginning in 1988, Albertario was positioned at exactly the right intersection of technical knowledge and institutional authority. He recognized early that digital acquisition was not merely a convenience — it was the future of clinical polysomnography.
The effort required more than swapping equipment. AASM (then known as the American Sleep Disorders Association) accreditation standards were written with paper in mind. Demonstrating that a digital system could meet — and exceed — those standards demanded careful documentation, rigorous calibration protocols, and sustained advocacy for a recording methodology that most in the field had never encountered in a clinical setting.
In 1990, Winthrop became the first AASM-accredited, fully paperless sleep disorders center in the world. It was a milestone with implications well beyond a single laboratory: it established that digital polysomnography was clinically valid, accreditation-ready, and scalable.
Albertario's role at Winthrop grew alongside the center itself. As Associate Director from 1999 to 2011, he was responsible for the design and oversight of a new $3 million, eight-bed facility — built to NYS Department of Health "Article 28" standards, AASM accredited, and Joint Commission accredited. Managing all aspects of that build, from floor plan to compliance documentation, was a years-long effort that cemented his understanding of what it takes to bring a high-functioning sleep center into existence from the ground up.
The paperless center work had a long tail. In 2006, Albertario presented "OpenXDF: Discussion on an XML-Based Open File Format for PSG" at a Philadelphia workshop focused on developing guidelines for the electronic interchange of polysomnographic data. The interoperability problem — how to share, archive, and analyze PSG data across different systems and institutions — was a direct descendant of the same questions he had been working through since 1990.
Digital recording had arrived. What the field still needed was a common language for the data it produced.
